ultrasound guidance for IUD placement

I am working with some ultrasound technicians who are adament that CPT code 76942 ultrasound guidance for needle placement.... is the correct CPT code to report ultrasound guidance for an IUD placement. Even though in one case the ultrasound was used only to confirm accurate placement. I advised them that a limited GYN 76857 or 769988 intraoperative guidance would be more appropriate.

Does anyone agree with the ultrasound technicians??

A follow-up visit in the office to check the proper placement of the IUD is reported as V25.42, “surveillance of previously prescribed contraceptive method, intrauterine contraceptive device.” If ultrasound is used, assign these codes:

Code 76857, “ultrasound, pelvic [nonobstetric], real time with image documentation limited or follow-up,” with V25.42
or

Code 76830, “ultrasound, transvaginal,” with V25.42
ACOG: Performance of an ultrasound to check IUD placement is not bundled into the IUD insertion (code 58300). But, keep in mind it is not a common practice to use ultrasound to confirm placement. Therefore, this would not be routinely billed. According to the ACOG Practice Bulletin, Number 59, “Intrauterine Device”, ultrasonography may be used to confirm the location of the IUD when the physician incurs a difficult IUD placement (e.g., severe pain, uterine perforation, etc.).
