Type 2 NPI Billing

Hello Everyone

How can we bill a claim under type 2 NPI? Can we bill on CMS 1500 from for the services they are providing and can we bill equipment on CMS 1500? Is type 1 NPI is necessary?

I am having these questions please help me.
