TUbe removal

Patient comes in with a T-tube in the right ear, been there for 7 years. The physician that removed the tube, was not the one who place the tube. I understand you cannot bill the removal due to it not being under general. Physician wants to know if he can bill an E/M code plus a removal of foreign body? I would say no, but need some back up. Although when I read dictation he did indicate a microscope was used and I can bill for the microscope? RIght? Thanks for all the input.
