Modifier 78 - 58 in Global

Patient had Hemilaminectomy 8/12/11 L5-S1 left L4-5 right L3-4 left. Herniated disc w/ stenosis.

Patient sustained several falls a few days after surgery after attempting to urinate her a pop and began having pain and radiculopathy. MRI confirmed recurrent
disk hernation at L4-5 and L5-S1 and was taken back to the OR for revision laminectomy foraminotomy discectomy posterior lateral fusion interbody spacer instrumentation auto and allograft.

This is a more extensive procedure resulting from the fall. I am confused as to whether to append 78 or 58.
