forgien body removal

I can't seem to get paid for both office visit & fbr. It's always one or the other.
I know a modifier 25 but that doesn't seem to work either. Should I be using modifier on the office visit of removal?

I use mod 25 on the office visit, and always get pd. depends on what code you use, if you use a code exp 65235,65260, and or 65265 you would also use a mod 57 because these codes have a 90 day post op. Codes 65205-65222 have no post op period, in this case you would only use a 25 mod.
