Leucovorin coding help

Hi,
How to bill Leuco if it's given with an initial chemo drug or a sequential chemo drug.

Oxaliplatin 10-12 96413
Leucovorin 10-12 ?

Avastin 96413
Decadron 96367
Oxaliplatin 96415
Lecovorin ??

I was told that if Leuco is given with a sequential chemo it should be coded as a sequential 96367 and + one additional 96366. If it's with the initial chemo drug (96413) it would then be a concurrent 96368. HELP

For the 1st one you would have a 96413 & a 96415 for the 1st and second hour of the Oxaliplatin
the Leucovorin would be billed as a 96368 because it ran concurrently the entire time with the Oxaliplatin.

For the 2nd one it would be dependent on the times that it ran but here is an example.
Avastin- ran for 1 Hour
Oxaliplatin & Leucovorin ran for 2 hours at the same time
Decadron ran for 20 minutes
===
96413 1st hr Oxaliplatin
96415 2nd hour Oxaliplatin
96417 Avastin 1 hr
96367 Decadron
96368 Leucovorin

Thank you. So it doesn't matter that Leuco ran with the sequential drug, it is still considered concurrent and should be billed as such. got it!

Yes anytime you have a drug that runs at the same time as another it would be considered a concurrent unless it's a push.
