CO 226 mcr denial code

Hi, what should we do if we get a denial from medicare "CO-226 N29"
Any help would be greatly appreciated

226 Information requested from the Billing/Rendering
Provider was not provided or was
insufficient/incomplete. At least one Remark Code
must be provided (may be comprised of either the
Remittance Advice Remark Code or NCPDP Reject
Reason Code.)
N29 Missing documentation/orders/notes/summary/report/chart

You could call your MAC Provider Services and see if they will tell you precisely what records they want. Otherwise, send relevant documentation, probably with a Redetermination Request, and reference the denial ICN#.
