Help coding a complicated home visit

Our Provider performed a New Patient Home visit for a very complicated patient. He documents in his notes that he "did a home visit on this new patient, approximately 2 hours; preceded by a review of her records, another 2 hours."

Not really sure of the coding on this one, maybe I am just overthinking it.

I am certain that this visit constitutes a 99345; however, I am not sure if I can code a "Prolonged Services with direct patient contact" code and/or "Prolonged Services without direct patient contact".

This is a new scenario for me, any assistance is greatly appreciated!
