Physical with cerumen removal and ekg

Hello - I billed a physical (99396) with cerumen removal (69210) and an ekg (93000). Modifier 25 was appended to 99396 and modifier 59 was appended to 69210. Diagnosis V70.0 was pointed to 99396 and 93000 and diagnosis 380.4 was pointed to 69210. The insurance bundled the ekg with the cerumen removal and paid nothing on the ekg. Any suggestions on submission of a corrected claim so that I can get the ekg paid would be appreciated. Thank you.
