Hospice billing-non employee
what is the correct CPT® code range for a provider to use when billing for hospice visits? These providers are not employed by the hospice house, which is free-standing. I have been trying to nail this down and it isn't easy. Most instruction is for the actual hospice house billing. So far I have determined that the codes would be outpatient. I have some info that says that the correct range is Nursing Facility Care 99304-99310, with POS 34.
I would like to bill these services with confidence.

What is the credential of the person doing the service? RN, DO, MD, PA??
What service is being performed, and where?
