skin tags

Doc removed 115 skin tags on the thigh area. The code is 11200 x 1 for the first 15 lesions. 11201 x 1 for each additional 10 lesions. So, that's 25 lesions total. Do I have to bill it per line for the 11201 9 times to add up the 115 or make 11201 x9 for one line?

Code 11200 includes 15 skin tags, you now have 100 skin tags left. Code 11201 is each additional 10 skin tags. The units for code 11201 would then be 10.
Code 11200 (1 unit), code 11201 (10 units). Diagnosis code 701.9. Medicare has an edit for MUE frequency for 11201, of 1 unit/day. Medically Unlikely Edits (MUEs) prevent payment for an inappropriate number/quantity of the same service on a single day. An MUE for a HCPCS/CPT® code is the maximum number of units of service (UOS) under most circumstances reportable by the same provider for the same beneficiary on the same date of service. If this is a Medicare patient send documentation or appeal if supported by medically necessary documentation.

Also, this seems like an unreasonable number for a patient's thigh.

I could not believe it either. I will double check with the doc about the # of lesions. Thanks for the info.
