CPT® code for removal of arm mass

op report says..... there was a large 3 x 5 cm fibrotic mass on the ulnar aspect of arm that was adherent to the bone. elliptical incision carried down through sbucutaneous tissue; it was excised and removed in its entirety.

path report says...... organizing granulation tissue with foreign body giant cell response and fibrosis; a neoplastic process is not seen.
