Discharge from hosp, admit to SNF

Hello,

I have a question regarding a discharge from the hosp and then the pt is admitted to SNF. The case that I am referring to is a hosp that has the acute side and also has SNF within the faculity. I have providers that think they do not have to dictate anther H&P for the SNF admission. They are using the H&P from the initial admit to the hosp, making hand written updates. I do not think this is correct and was wanting some input. If anyone knows of any websites that can confirm documentation requirements---I would appreciate the info.

There are different criteria for each admission. The SNF requires RAI, as one criteria.

but...

the requirements on that link are for SNF payment, not physician payment. For physician billing, you could consult the documentation requirements for the level coded by the doc. It is common for docs to use the hospital H&P and write an addendum to update the history and physical. That would be the lowest level SNF admit code. That being said, the SNF may require more by their own rules.
