nursing home admissions

I can't find anything that tells me if billing 99304-99306 it has to be a full yr from the last one. I know that is a patient has been discharged or in the hospital you can bill these codes again when readmitting to nursing facility.

nope

they could be readmitted every 20 days and get a new initial visit. The yearly SNF H&P code can only be once yearly.
