Modifier QW

I have conflicting information as to when modifier QW should be used for CLIA tests.

Should it be on
1) all Medicaid plans (Ohio)

2) commercial insurance plans

THE QW Modifier is placed on lab tests that are clia waived.

There is a list that you can find at the link below;

Does anyone know if the modifier applies to Medicaid HMO's or just the standard Medicaid? Unfortunately with the billing outsourced we don't see the EOB's to know one way or the other. Thanks!

As a standard, if it applies to Traditional Medicaid, it would apply to the Medicaid HMO. You would need to check the Medicaid HMO fee schedule or the Medicaid HMO bulletins to be sure.
