What is the difference between CAD 414.00 and ASCVD 429.2

It is unclear to me what the difference is between 414.00 Coronary Atherosclerotic Disease and 429.2 Atherosclerotic Coronary Vascular Disease. Our biller is telling us these are different diseases. Any clarifications?

Is the issue for a diagnostic test or an office visit? In what context is your billing questioning your diagnosis code?

The ICD-9-CM diagnosis codes for CAD are classified to subcategory 414.0x. Coding CAD requires documentation of the coronary artery involved and whether or not it is of a native artery, an artery of a transplanted heart, or a bypass graft. The fifth-digit subclassification identifies the coronary artery involved.( Coronary atherosclerosis), specified.
Code 429.2 - Ill-defined descriptions and complications of heart disease – cardiovascular disease (unspecified). The key word is "unspecified".

I would use

CAD 414 for the patient with established coronary artery obstruction, either by catheterization or other imaging like CT angiogram. I see ASCVD as more ill-defined to include all the diseases that occur when your arteries start to clog (AtheroSclerosis) and stiffen, and would include hypertension, peripheral vascular disease, carotid artery disease, aortic aneurysm and coronary artery disease.

Personally I have never coded a patient 429.2 as it is too vague.
