Pronoucing death

Is there a dx code for the prouncing of death, or a specific cpt & modifier?

use the discharge codes for CPT and the underlying illness as the ICD. The physician declaring death is the only doctor that can bill the discharge day services code.

Death pronouncement

It's actually not easy to get paid for this. If the physician has already seen the patient in the hospital that day, there's no separate billing for the second visit. (One E/M per day)

If the physician hadn't seen the patient yet, and was called in, and saw and pronounced the patient, bill whatever category of code is appropriate, most often, discharge from hospital. (Assuming the patient was in the hospital.)

If the physician didn't go to see the patient, but just filled out the paperwork later, there is no billing for the service.

Here's what the Medicare Claims Processing Manual says:

E. Hospital Discharge Management and Death Pronouncement
Only the physician who personally performs the pronouncement of death shall bill for the face-to-face Hospital Discharge Day Management Service, CPT code 99238 or 99239. The date of the pronouncement shall reflect the calendar date of service on the day it was performed even if the paperwork is delayed to a subsequent date.
