Diagnosis Code 362.02

Hi,

I'm using cpt 67228 w/ diagnosis code 362.02 and mod RT. These are being rejected by Medicare as "Missing, incomplete, invalid dianosis or condition. Must code to the highest level of specificity.

Any help on how to get these paid would be appreciated. Thanks!
