is this appropriate - 99213 modifier 25 and G0438

in terms of determining how to incorporate teh g0438 into workflow, i've decided that for all my chronic car patients who come for their 4th routine chronic codition followup, to make the visit both routine fu and perfrom the AWV g0438 using a template. I then make a standard 99213 note for the encounter and I manually complete a AWV form by hand I got from the AAFP website and scan it to their chart.

I plan on doing this for ALL my medicare patients. Is this accetable by medicare?

If it's ok, CMS says that no parts of the AWV can be used in the 99213. now that makes it kind of hard to document if the patient is there for a chronic disease visit since DM2 will be in my 99213 PMH and DM2 will also be listed under updated list of chronic conditions in the AVW. Will medicare allow this scenario?

absolutely! You have a great system. The biggest part is to be sure the assessment and plans are clearly separated, which your form could accomplish
