Are they billing A9270 to the patient pharmacy benefit?
Are they billing Medicare Pharmacy( Plan D)? what cpt?

Are you billing out pt- per pulication 11333? What CPT do you use?

Is your doctor providing a self administered medication at an office visit? I don't believe that is separately billable.
