Procedure code 45378

I need to know if the diagnosis code 211.3 is payable for this procedure and if not what would be the alternative for this diagnosis?

Yes, I think so...

I have billed 45378 and 211.3 together before. There are many reasons why a doctor might encounter a polyp but not remove it. But you should still report that the polyp is in there. So far, we've always been paid for it.
