ICD-10 PCS code for skin graft.

According to ICD 10 guideline B3.9 -- If an autograft is obtained from a different body part in order to complete the objective of the procedure, a separate procedure is coded for that excision. I had a practice question in which the skin was taken from the thigh to replace skin removed from the lip. The answer showed a code for the excision of skin from the lip and replacement of skin to the lip. No additional code for excision of skin from the thigh. Skin is listed as many different body parts in the PCS guide so I assume that skin excised from the thigh to be used for the lip would require a separate excision code???
