what is the proper way to bill texas healthspring for a physical

i have recently noticed that payment to physical test have been denied even though we have billed the office visit with modifier 25 and modifier 59 on the physical. i have faxed a corrected claim and i have removed the modifiers and still gets denied. does anyone know what is the proper modifier to use?

but bill the physical with no modifier and the E&M with -25. That's how 49 other states accept it, assuming the patient has coverage for both services.
