Coding procedures with office visits

I'm finding that I'm getting denials when I code procedures with office visits. I see that there are CCI audits on most of the procedures we do here in our ENT office. Are other offices just coding the procedures when they are done on the same day as the office visit. It doesn't seem to matter that modifier 25 is being used. What are othe offices doing to get paid?

I work in an ophthalmology clinic and if we do a minor procedure in the office, we just bill the procedure as the exam is included in the procedure. If they come in for a different dx, say glacoma follow up, and punctal plugs are done for dry eyes, then we will bill the exam with the 25 modifier and the procedure code.
