Medicare: Yearly Physical

When billing for yearly physical "G0439" , do I also use a preventative(i.e. 99396)?

nope- don't risk it by including that code; and for clarification, it is not a yearly physical- it is a yearly wellness visit. Technically the patient never needs to be touched by the physician- it's all history except vital signs.

I agree not to bill both however office visits are acceptable i was told.

So a patient seen for other problems plus a a wellness is done, do I append a mod "25" to an E/M code for the other problems? And what would be a diagnosis code that can be used for the wellness?

Yes 25 modifier. I use V70.0
