I need help in determining the correct place of service for a sleep lab located off site. POS 81 has been used and denied by Medicare. Place of service 11? I would appreciate any assistance with this one as we have many denied claims.

boy is that a tough one. I will be curious to find out the answer. But what I am thinking is who owns the lab? Is it a lab or a clinic? Does it have it's own NPI? Is that NPI conneted to a physician or a hospital? I am wondering if it is connected to a physician if it could be POS 11, or 21 if connected to a hospital.
I think it can only be 11, 21 or 22 but the description for those POS codes does not seem to fit. Hmmmmm....

Sleep lab is an extension of a hospital just located off site. Traveling clinic physicians work this sleep lab. I have been unable to get a definitive answer while claims are waiting. We are located in Illinois.

Any thoughts are welcome and very much appreciated.
