Medicare is denying only one visit or consultation per physician per day is covered.

I have a clarification here:

1. If the physician's primary specialty is different and sub specilaty is same still Medicare will deny the claim?

2. If the physician's primary specialty is different but belongs to same group, sub specilaty is same still Medicare will deny the claim?

Please advise on this

What specialties and subspecialties specifically? Sometimes, what we think of as a subspecialty designation isn't designated that way by Medicare.

Same diagnosis code?

Coverage?

For example if the physician has cardiology and Internal Medicine as their specialties, the physician must be having cardiology as primary specialty and Internal Medicine as sub specialty.
