Diagnostic Trauma/Injury Coding

In diagnostic coding, can you please tell me the correct way to code Trauma/ Injury codes if the clinical indication states "Trauma" or "Injury" and is not site-specific ?

For example: Patient comes in to ER for Reason of: "Trauma". An Xray is performed of the RT Ankle, and it is normal. Which is the proper way to code it from these examples:

1)V71.4, 2) E988.9 (or E928.9) OR 1) 959.9, E988.9 (or E928.9) OR 1)V71.4, 2) 959.9, 3) E988.9 (or E928.9)

Also, can you please indicate your resources/guidelines that you are referencing for your answer?
