Skilled Nursing Facility Followup from Hospital Stay

Our physicians see patients in the hospital and then followup with these patients in a skilled nursing facility. This skilled nursing facility is affiliated with the hospital and is actually located on the same grounds as the hospital. Should we bill for these services using procedure code 99212-99214 with place of service 31 or should we use the skilled nursing facility procedure codes 99304-99310? Additionally, should our services for Medicare patients be billed under consolidated billing Part A through the nursing facility?
