Very good article. I think it's important for providers and clinicians to read this. Very hard at times to understand what was done at the current vist and what was from previous visits. I liked the paper charts as at least you could see what was current and what had been done previously.

I will have to wtach for your blogs.

Thank you. The time line can be confusing if the HPI is "carried forward."
