medicare secondary payer and consults

One Carrier is teaching the following:
-Providers may bill primary payers using the consultation codes, but would need to re-code the service to the appropriate E&M visit code to bill Medicare as secondary
or, Alternatively, providers may consider billing the appropriate E&M visit code to the primary in order to facilitate the MSP payment.

MSP and consults

It is shocking to me that they are allowing physicians to "re-code" but on a practical level, how are you going to do this? Does anyone have a plan?

My understanding is that these claims will go electronically from the private to Medicare for the secondary consideration. They'll be denied (because they have a consult code billed) and we will have to re-code and re-submit. On a nuts and bolts level, how are you going to do this?

My feeling exactly--words are one thing, performance is another. MSP on normal claims are a headache, I cannot believe this can be managed efficiently by Medicare but will instead be mis-managed. We'll see. NGS is teaching this in an audio conference.
