99195-Phlebotomy?
Hi,
Can somebody tell me what is the common use of 99195? If we can add CPT® code and E/M code at the same time without any modifiers used?
Just like for e.g.
DX code: 607.84 , 401.1 , 715.16 , 692.9.
CPT® code: 99214 and 99195
Is it necessary to used 99195? And TOS of 99195 is 5-lab? Or should I use 36415?

Phlebotomy is the draining of blood for therapeutic purposes (modern day blood letting). Most common for hemochromatosis with iron overload. It is not the same as drawing blood for a lab test-36415

If patient came in, just to had a blood work? What code should I used? Lot of patient came in just to have the blood work

Do I still need to use e/m code 99211. Nurse is the one who did the lab draw? And what are common icd 9 code if the patient came in just to have the blood draw?

36415 for blood draw, not 99211. The diagnosis would be the condition requiring the blood work.

Yes.. 36415 is the appropriate CPT® code for lab draw. At the same time can I use 99211?

Only with adequate documentation meeting the CPT® requirements (in addition to the draw).

Not unless a separate identifiable and medically necessary visit was performed and documented. Why 99211?

And a caveat: The "nurse" visit will most likely be assessed a copay. So, you should warn the patient ahead of time, or risk bad will and phone calls on the back end.
