Medicare Annual Wellness Visit

I understand using the new G codes...but what should be used for diagnosis codes? I am already getting denials for using V70.0. Are we to code specific chronic diseases addressed?

I have scoured the literature- CMS has said nothing about the ICD-9 to use. DId you bill just the V70.0 and the G code?
