re: CPT® 11601

Go to Medicare's website, to the coverage determination section.

https://www.cms.gov/medicare-coverage-database/

The trick is to know the search term that CMS has used, not always easy. I don't believe there is a national coverage for lesion removal, but one from NHIC can be found here:

http://www.cms.gov/medicare-coverage-database/indexes/lcd-list.aspx?Cntrctr=211&name=NHIC%2c+Corp.+%2814502%2c+MAC+-+Part+B%29&ContrNum=14502&CntrctrType=MAC%20-%20Part%20B&s=55&bc=BBAAAAAAAAAA&#ResultsAnchor

It is called removal of skin lesions. I found it by going to NHIC's Medicare website, looking at the billing section, and just looking at the list of policies. I found one for removal of skin lesions. It lists indications.
