Central venous line

I am new to surgery coding and I have an extravasation from the venous port and they are removing it from the left side and are putting in a new one on the right side. My question is will this be indexed as a replacement of the venous port or will it be a removal of left port and insertion of a rt port. the CPT® codes keep throughing me off because it keeps saying same site and in by brain because it is going from a left site to a right site it is 2 different sites. Please help....Thank you in advance!!!!!!

The replacement codes can only be used if the catheter is being replaced through the same venous access site. If a venous access device is placed in another site, then the appropriate placement code should be used.

Use Insertion code for new site. Removal from another site makes this 2 sites, nor same site.

Just for clarification I would use a CPT® code for the removal of the old one and a CPT® code for the insertion of the new one????

If performed, yes. Modifier -59 on removal
