NURSING HOME VISITS by consultant

I work for a neurology group and need to confirm some information.
After a patient has a stroke they may be in rehabilitaiton for the stroke in a nursing home. Sometimes we have seen the patient in the hospital prior to the nursing home assignment and sometimes we are asked to evaluate patients who we have not seen previously. My question is assigning E/M codes.
Do I use Nursing Facility Care Codes (new or established) 99304 -99310 with a place of service code 31, or weather to use office codes with a place of service code of nursing facility.

Our doctors routinley visit this nursing facility for all stroke patients. Also would there be a difference if a patient is a resident of the nursing home or just on a part A stay. Thanks for any guidance.

Use the 99304-10 codes but remember if you are called to see the patient by a primary care doc, then the first visit is a consultation if non-Medicare' place of service is skilled nursing facility- 32
