nursing home admission

Hello all,
If I see a patient in the hospital or in the office - hence a existing patient, and then follow them into the nursing home, do I bill a nursing home admit code (99304-99306) or a nursing home fu (99307-99310).

every time they are admitted you can bill the admit code. Your relationship does not count, same as hospital admissions

coding nursing home readmits

If an nursing home patient I admited recently, gets sick and has a 24hour hospital stay, when they come back - do I bill a admit code or is that just a followup. Is there any different documentation requirements for readmits or is there no such thing as a readmit from medicare's coding POV (point of view).
