Can you bill an E/M visit with this code?

More detail needed. Was an E/M medically necessary above and beyond the destruction of the lesion? Where was service rendered? Was a new patient or established patient? What diagnosis code(s) were assigned? It must be clearly documented in the medical record to show that a significant, separately identifiable service was performed, in addition to the minor procedure code. Minor surgical procedures and global procedures include preprocedure, intra-procedure, and post-procedure work inherent in the procedure, the provider should not report an E&M service for this work.
