"No Charge" for Outpatient vs 99201

Does anyone have a reference for information re how to identify what services/procedures are "no charge" vs billing 99201 for outpatient? Thanks.

99201 is for a new patient visit. It is used for a simple visit with low complexity and little history, like... patient-"I have a mole." doctor-"That's a skin tag. Ignore it."
