Billing for sick visit and a screening exam in one setting

I am coding/billing for a rural family practice group who sees patients for a "sick visit" and codes appropriately, however, if the patient asks for a PSA screening (for example) or any other "screening exam" during the "sick visit", how is this coded? Do you bill for both sick visit and preventive on same day and modify with -25 on sick visit (office visit) and then list the preventive code which you may or may not get reimbursed for (depending on payer) and then the screening PSA CPT® code with the Vcode ICD-9? Or is there another way to include this in the sick visit and get paid for the PSA? We cannot attach a regular ICD-9 to the PSA screening as we do not want to document a problem.

Can I just bill the sick visit and bill PSA (Medicare or non-Medicare) along with primary v code and get reimbursement? Example: 99213-401.9

Sick visit and preventive visit

I bill sick visit with a 25 modifier and the preventive med visit with the appropiate code 99381-99387 or 99391-99397. V7644 does not go with a diagnostic PSA. For a psa screening I use G0103 to Medicare with V76.44.

Sorry about the incorrect ICD-9 code for the diagnostic PSA but if during the sick visit, can we just bill that screening psa with the v code and get it paid without billing a preventive E/M along with it?
