Prolonged skilled nursing facility visit

When I bill Medicare for a prolonged nursing home visit, they pay for the CPT® CODE 99310 but don't pay for CPT® 99354 covering the additional time spent face to face with the patient. It is not the initial visit but a subsequent visit to the patient. The doctor spent and hour and a half with the patient. Any suggestions on how to get this additional time paid?
