Can I file in the chart?

Can we retain patient assist foundation applications in patient's charts?

The patient's chart is a legal medical record. Financials should be elsewhere

We keep those things in the chart

It does not concern our charges or billing so we keep it in the chart and we need to refer to it every 3 months to reapply for the patient.
