medicare billing and documentation for annual well visit

if a medicare pt makes an appointment for a physical must we code it as a awv G0438.
if we perform a full physical exam ,can we use part of the exam not listed in the awv requirements to support an additional sick visit is the provider is treating another issue?
if we offer the awv but the medicare pt. perferrs the cpe can we bill the cpe .Medicare has stated the AWV is not a CPE.
I have asked yjese questions of Medicare however their answer was not clear.

where to start...

an AWV is not a physical - in fact there is no requirement to actually touch the patient except for the BP. If they have a problem, you can also do a 99213 and the physical exam can be counted for both a problem and well visit. The discussion and plan must be separate.
If the patient wants a "full physical" then you code a well visit and can charge for a problem visit. The payment on that is complex- I can't explain it
