Is ok to count a "Family history" when the doctor only documents

What does that mean? Does it mean "irrelevant"? Does it mean the provider even asked or thought it was irrelevant to presenting problem and did not ask? Irrelevant -not important, pertinent, or germane to the matter. Too gray a statement and will be based on eyes of the beholder. I would say, if the family history was indeed questioned, to clearly state the findings. might take a few seconds!

or, "reviewed, not pertinent" to document that it was performed as opposed to not even asked
