94640, 94664, modifier 59 and 76

If my Provider does a breathing treatment 94640 a pulse oximetry 94760 and repeats the 94640 do I use a modifier 59 on the first 94640 and a modifier 76 on
the 94664

um

94060 is pre and post bronchodilator so you can only bill once. 94664 is for teaching MDI use. Pulse oximetry does not get reimbursed if any other code is billed.
