Testosterone injection (cpyionate)

Biller is saying Medicare will not reimburse for testosterone. There is a J code for this drug - J1071 per 1mg. Is it appropriate to use this for Medicare? Would the charge and/or reimbursement then be 200mg ( 200 x 0.03$) or $6 for a 200mg dose?
