Patient seen in office 2 different times same day

Does anyone know: We saw a Medicare patient for a wellness exam. During the exam the doctor treated 6 actinic keratosis. Then some time later in the day he returned and the doctor did an excision biopsy on the back of his neck. I am wondering about which modifier to use for the second visit. Is it 76, or maybe 79 since the 17000/17003 were done prior during the day? Any help with this will be greatly appreciated. Thanks

You submit one bill as if it was one visit with the appropriate modifiers for a procedure on the same day as a visit
