33 modifier

do you use the 33 modifier on all the lines on the claim if the screening becomes diagnostic or just the 1st line of the claim? For example 45385 45384 45381 45382 are all billed with dx V76.51 211.4 and 211.3. Would you use the 33 and PT together?

You would append the modifier 33 to each of the colonoscopy codes that were screening turned diagnostic. You would not use the 33 and PT together. Modifier 33 is for commercial insurance claims, and the modifier PT is for use with Medicare, Medicare replacement plans, and Medical Assistance.
