I know that literature went out in 2009 stating hearing/eye exams no longer need to be included in physical exam. Unfortunately, I can not put my hands on it. Could someone please reference where this information can be read?

Are you refering to Medicare patients? Children? School physicals?

The AMA added commentary to the 2009 CPT book that stated that the visual acuity test 99173 should be paid separately and payment for it is not included in the payment for the preventive medicine services. You can find it in the CPT book at the start of the section on preventive medicine, and in their 2009 CPT Changes, Insider's View (whatever it is called: I'm out of the office.)
