Opthalmology Coding Question

Hello,

We are billing for ophthalmology ambulatory surgery center (POS 24). We are billing cataract surgery code 66984 along with Z7506, Z7514, Z7500, Z7512, V2632, & X7700, but we are getting denial from Alameda Alliance. For CPT® 66984 they denied as claim is missing data required for adjudication and for the rest of claims they denied as invalid procedure code. Can anyone give me correct coding.
